Recognition of disability by ANPH

THE REQUIRED DOCUMENTS TO BE PLACE IN A GRADE OF DISABILITY


- The comprehensive assessment application form to be placed in the degree of disability is drawn up according to the model in Annex no. 4 of GD 430/2008.
- The comprehensive assessment application form can be obtained from the websites of the county, and respectively local General Directions for Social Assistance and Child Protection from Bucharest, hereinafter referred to as DGASPC, FROM DGASPC, from the local hall and form other sources.


The documents required for the complex assessment are the following:

- Complex assessment application form ;
- Copy of the identifying documents;
- Medical records;
- Social survey carried out by specialized social services in the hall, in whose radius has the domicile or residence of the disabled person, according to the frame model set out in Annex 6.


The medical documents referred to are:

  • Report on current medical situation, drawn up by the medical specialist
  • Medical letter form from the family medical officers, according to the model in annex 5, only in case of the first presentation at the department of complex assessment
  • Laboratory investigations requested by the department of complex assessment.

 
The complex assessment application form, accompanied by the necessary documents:

  • Is submitted by the demanding person to the Hall Registry from the locality of domicile / residence or to the DGASPC Registry
  • In case of persons requiring a reassessment, is submitted 30 days before the expired term of validity for the certificate of disability degree inclusion.
  • Is sent to the department of complex assessment, compulsorily, within 5 working days of registration, by the hall town, and within 24 hours of recording by the DGASPC.


The department of complex assessment shall verify and review the file of the requesting person, which must contain the above documents, and within 5 working days, notify to the person concerned the planned day on which he (she) is going to attend in order to be assessed; if a file is incomplete, it shall require the file completion with the necessary documents.


The complex assessment is made in maximum 60 days from the application and documents registering at the department of complex assessment.


In case of persons unable to walk, based on the medical letter and social investigation, the complex assessment shall be made at the domicile / residence of the person concerned.
