DOCUMENTS NECESSARY IN VIEW OF SUBMITTING THE SPECIAL AID FILE FOR ADULTS WITH ACCENTUATED AND SEVERE DISABILITY
ACCORDING TO LAW 519/2002:


• Application
• Copy of the certificate for subsuming in a category of persons with accentuated or severe disability
• Copy of the identity document of the disabled person
• Standard statement according to the model found at the registry of Directia de Protectie Sociala (Social Protection Directorate)
• Copy of the retirement decision and of the last retirement check (if applicable)
• Other income evidence, function of the case
• Other documents for completing the file, function of the case.

 


DOCUMENTS NECESSARY IN VIEW OF SUBMITTING THE FILE FOR DOUBLE ALLOCATION FOR DISABLED CHILDREN:

 
• Application
• Copy of the certificate for subsuming in a category of disabled persons who require special protection
• Copy of the identity document of the disabled person (identity card/birth certificate)
• Copy of the identity document of the legal representative who drafted the application
• Receipt issued by the authority who paid previously, which indicates the last month for which allocation was paid
• Copy of the last double allocation check (in case he/she has already beneficiated).


DOCUMENTS NECESSARY IN VIEW OF EXEMPTION FROM THE PAYMENT OF RADIO AND TV SUBSCRIPTION FOR PERSONS WITH SEVERE DISABILITY (IN CASE OF BLIND PEOPLE WITH SEVERE DISABILITY, ALSO EXEMPTED FROM THE PAYMENT OF THE ELECTRIC POWER SUBSCRIPTION:


• Application
• Copy of the certificate for subsuming in a category of persons with severe disability
• Copy of the identify card of the disabled person
• Copy of the last Electrica invoice.

In case the invoice is issued on the name of another person, different from the disabled person, documents which indicate kinship, as well as the evidence that they reside at the same address will be submitted.


DOCUMENTS NECESSARY IN VIEW OF OBTAINING PRIORITY IN INSTALLING THE TELEPHONE STATION AND EXEMPTION FROM PAYING PHONE SUBSCRIPTION WITH 100 INCLUDED PULSES (400 FOR BLIND PEOPLE) FOR ADULTS WITH ACCENTUATED AND SEVERE DISABILITY:

• Application
• Copy of the certificate for subsuming in a category of persons with accentuated or severe disability
• Copy of the identity document of the disabled person
• Copy of the last telephone invoice.

In case the invoice is issued on the name of another person, different from the disabled person, documents which indicate kinship as well as the evidence that they reside at the same address will be submitted.

DOCUMENTS NECESSARY IN VIEW OF OBTAINING GRATUITOUSNESS FOR PUBLIC TRANSPORT MEANS, OPTIONALLY, WITH RATB MEANS OR WITH THE SUBWAY FOR PERSONS WITH ACCENTUATED AND SEVERE DISABILITY, ACCORDING TO LAW 519/2002:

• Application
• Copy of the certificate for subsuming in a category of persons with accentuated or severe disability
• Copy of the identity document of the disabled person.

For personal assistants of severe disabled persons, employees of Directia de Protectie Sociala, it is necessary to submit only an application which indicates the option: transport with RATB or subway means.


DOCUMENTS NECESSARY IN VIEW OF OBTAINING GRATUITOUSNESS FOR INTERURBAN TRANSPORT, BY TRAIN OR BUS FOR PERSONS WITH ACCENTUATED AND SEVERE DISABILITY, ACCORDING TO LAW 519/2002:


• Application
• Copy of the certificate for subsuming in a category of persons with accentuated or severe disability
• Copy of the identity document of the disabled person.

In view of obtaining or modifying train travel permit, two photos of the identity card type are submitted as well as the old permit issued by Inspectoratul de Stat pentru Persoane cu Handicap. (State Inspectorate for Disabled Persons)
